Dr. Chantal Dufour

I will be leaving my practice at City Center Family Physicians to pursue hospital based work. The decision to leave has certainly been a difficult one and I am grateful for all the wonderful people I have been able to meet while working at CCFP. It has been a great pleasure to have provided you with medical care over the last 3-4 years.

Unfortunately, I have not been able to secure a new family physician to take over my practice and there are no physicians at CCFP currently accepting new patients. Clinical information may be sent to another clinic upon request. There is a fee for transfer of charts.

My last day in clinic will be October 27 2022. Patients will continue to be able to access medical care at CCFP until November 3, 2022.

Dr. Matthew Kushneriuk Relocation

It is with mixed emotion that I am relocating my medical practice as of September 19th, 2021. This decision has not been made lightly, as I have enjoyed working at City Centre Family Physicians for the past five years.

With many thanks to my colleagues, I have been able to ensure that all of my patients will have continuing care with a physician at City Centre Family Physicians. 

Please contact the office to either book an appointment or confirm which physician (previously selected by Dr. Kushneriuk) you will be switching to before September 13th. 

However, if you prefer, you may obtain the services of another physician outside of CCFP. If you choose to do so, I recommend proceeding as soon as possible to ensure a smooth transition for your health care. The Saskatchewan Health Authority keeps a list of physicians who are accepting new patients at https://www.saskhealthauthority.ca/Services-Locations/doctors-accepting-new-patients

Your medical records are confidential, and if you so choose to leave CCFP these records may only be released through your consent.  I will be pleased to provide a summary of my care while you have been my patient, and with your consent, will arrange to have a copy of your file transferred to your new physician’s office.

It has been my great pleasure to have provided you with health services in the past, and I am grateful to have had the opportunity to meet some wonderful people throughout my time at CCFP.

Dr. Steven Goluboff’s Retirement

City Centre Family Physicians is announcing the retirement of Dr. Steven Goluboff after a 40 year affiliation with this group.  His practice will be taken over by University of Saskatchewan graduate Dr. Daniel Fahardi, who will join the group at the beginning of August.  Dr. Goluboff would like express his appreciation for the privilege of taking care of all of his patients over the last 40 years. 

Corona Virus (COVID-19) Information

If you have travelled and are concerned you may have corona virus, please call the SK Health Line at 811 and follow their advice.  If they advise you to see your physician, do not present to the clinic but instead call the clinic to book an appointment specifically for the corona virus.  You will be given further instructions over the phone from clinic staff.

CCFP’s NEW NO-SHOW POLICY

CCFP now has a no-show policy. If you miss your appointment or cancel within one hour of your appointment time, there will be a fee. Your missed appointment means that someone else cannot be seen instead and this negatively affects our wait times.

Regular appointment: $50.00

Vasectomy or circumcision: $150.00
